What paperwork should my new employee complete?
Have them complete the New Employee Packet. The completed packet should be sent to Fiscal Assistance by either fax, email, or mail. Fiscal Assistance will complete a criminal background check and review all paperwork. Please wait for Fiscal Assistance to set a start date for your new employee before you allow them to work for you.
My employee is a minor under 18 years of age. Is there anything additional they need to complete?
To be an employee, you must be 16 years of age. Work permits are no longer required when you are 16 and older.

What paperwork do I need to complete to become an employer?
To enroll in Fiscal Assistance’s Employer Agent program, you should complete the New Employer Packet. The most up-to-date packet can be found on our website. The completed packet should be sent to Fiscal Assistance by either fax, email, or mail.
I have a (financial and/or healthcare) power of attorney. Who signs the new employer paperwork?
You, as the employer, not the Power of Attorney should sign the paperwork. An ‘X’ or hand over hand signature is acceptable.
The IRS Forms SS4 and 2678 should only be signed by the employer or the court appointed guardian, not the power of attorney.
I have a court-appointed guardian. Who signs the new employer paperwork? What documentation does FA need?
The court-appointed guardian should sign the paperwork. FA and the IRS requires a copy of the final court guardianship paperwork be submitted with the new employer packet.

A fillable PDF timesheet is available to complete and print off. Please be sure both employer and employee sign and date each timesheet. Download the fillable PDF timesheet.

How do I change my direct deposit information?
Complete a new Direct Deposit Authorization form. Attach a copy of a voided check or account verification letter from your bank. Send the information by either fax, email, or mail.
I need a copy of my paystub, how do I get one?
I need to list my employer. Am I an employee of Fiscal Assistance, Inc.?
No, your employer is the person you provide supports to. Fiscal Assistance is your employer’s fiscal agent.
I need an employment verification completed, who does that for me?
Fiscal Assistance can complete an employment verification on your employer’s behalf. We do not verify employment over the telephone. All requests should be faxed, emailed, or mailed. All requests must include a dated and signed release from the employee.
When requesting a verification, please allow plenty of time for us to complete it. Our goal to respond in within 24-48 hours.
When will my W-2 be issued?
W-2’s will be mailed by January 31st every year.
